To be eligible to participate in athletics, each student must: (1) maintain good citizenship by following school rules, (2) maintain good work habits.  All athletes must have proof of health insurance.  Ed Code 32221.5 requires all members of a school athletic team be covered by accidental injury insurance.  For athletes without health insurance, the office has information on affordable policies. 

Here are the forms you need to complete and submit for your student to participate in sports. To participate in sports all students must have the following forms on file through register my athlete; all of these forms are available in the office and must be downloaded through register my athlete.

  • Sports Physical Form: Physicals must be signed by a licensed physician stating that he/she is physically fit to participate in athletics. They are good for three years so that incoming sixth graders can participate all three years at ACMS with the one physical unless there are health changes and/or injuries. Physicals must be completed before students can try-out for a sport.

  • Concussion Form: This form is good for one year; it must be filled out, signed and submitted each year before students can participate.

  • Sports Authorization Form: This form is good for one year; it must be filled out, signed and submitted each year before students can participate.

  • Sudden Cardiac Arrest Form: This form is good for one year; it must be filled out, signed and submitted each year before students can participate. This form is new this year.

  • 10/29 Parent Meeting, ACMS Gym, 5:30-6:30pm. All Parents: Be There!
  • 11/2 Truckee Ski and Snowboard Swap 11am-4pm.
  • 11/9 Tahoe Cross Country Nordic Ski Swap (the best ski swap in the area for Nordic gear) 9am-4pm.
  • 11/13 and 11/17 Equipment and Uniform Days @ ASC. (RSVP Link TBA via email.)
  • 11/22 All paperwork and checks are due to the office. (Since we don’t have try-outs, this will determine who is on the team.)
  • 12/4 First Practice @ ASC Training Center
  • Check our team app (Spond) for subsequent Practice Schedules.

  • Register for ASC Training Center ACMS Trail Pass
  1. Go to asctrainingcenter.skiclubpro.team/user/login and create a household account.
  2. Fill out all the required information for your child.
  3. Go to the special team link at asctrainingcenter.skiclubpro.team/registration/115/start and enter password ACMSNordic24
  4. Register! A confirmation email will go to your address.
  • Download the Spond Sports Team Management app to your phone or device (we will use this for schedule communications)
  • Turn in sports team donation check for $100 made out to ACMS
  • Make sure to complete the Register My Athlete information at acms.ttusd.org/athletics/register-my-athlete
  • Turn in forms to ACMS; Forms can be accessed at acms.ttusd.org/athletics/sports-policies-and-procedures
  • ACMS Concussion Form (complete annually)
  • ACMS Student Sports Participation Authorization Form (complete annually)
  • ACMS Sports Physical Form (complete one time while your student is at ACMS)
  • ACMS Sudden Cardiac Arrest Form (complete annually)
